03 AUG 2004 USAF Beech 1900 crashed caused by pilot's sudden cardiac death [to table of contents]
Investigation revealed that the March 16 crash of a USAF Beech 1900 at the Nevada Test and Training Range was caused by the pilot's sudden cardiac death. It appeared that the pilot violating federal policy and directives, willfully deceived flight medical examiners, suppressed significant medical information and ingested inappropriate medications for a deteriorating and dangerous health condition. The Flight Safety Foundations supports a draft ICAO resolution to develop legal guidance for member states in enacting legislation to `effectively protect information from safety-data-collection systems, both mandatory and voluntary, while allowing for the proper administration of justice`. The FSF says accident investigation - not criminal prosecution - should be the first priority, and safety data collected under voluntary programs or mandatory programs should remain privileged information. 08 AUG 2004 Nosegear of ATR-72 collapses on landing [to table of contents] A Thai ATR-72 operating as flight TG198 from Chiang Mai suffered a nose gear collapse on landing at Mae Hong Son, Thailand. East African Safari Air from Kenya has been suspended from flying into Italy by the Italian Civil Aviation Authority because of faulting technical operation standards. One of the airline's Boeing 767 planes is still grounded in Rome following an engine fire that resulted in an emergency landing on July 28. A crash involving a Canadair Challenger at Birmingham January 2002 that killed five was caused by the crew's failure to de-ice the wings before takeoff, accorinding to the report released by the AAIB. Possible impairment of crew performance by the combined effects of a non-prescription drug, jet-lag and fatigue were considered factors. (AAIB)

20 AUG 2004 Passengers injured as Azores plane avoids collision [to table of contents] Some 34 passengers were injured as the pilot of a TAP Air Portugal Airbus 310 carried out an evasive manoeuvre to avoid a collision with a smaller plane. Dubai-based Emirates Airlines has installed Braille and extra-large font Safety Cards on all of its flights. Additionaly all cabin crew members will undergo specific training to enable them to assist passengers challenged by any impairment. Beira Airport in Mozambique was closed for two ours after a Transairways Embraer 120 was involved in a landing accident. None of the 3 crew and 28 passengers were injured but the plane, operating on a flight for LAM, suffered serious damage to the undercarriage. (Agencia de Informacao de Mocambique)
